Patient Forms, On-line Payment, and First Appointment

Patient Care Specialists are happy to answer questions regarding your first appointment, insurance verification and payment methods.
 

Please arrive 15 minutes early on your initial visit and bring the following:

  • Prescription/Referral for Physical Therapy-
    • Referral from a physician, surgeon, podiatrist, chiropractor, nurse practitioner, physician assistant or dentist–Providers often have internal forms or can use our form.  Required by some insurance, but Missouri Direct Access allows 10 visits for up to 30 days without the referral.  Call us at 816.524.7040 for specific information.
  • Patient Forms (see below)
  • Insurance card(s)  In-network Insurance here.
  • Picture ID (if an adult)
  • Wear comfortable clothing
If possible, please print and complete the patient registration forms prior to your initial visit.
